Stories That Matter
For one person, progress may mean feeling confident enough to join a community activity. For another, it may mean learning a new routine, making a decision independently or feeling comfortable with a familiar support worker.
We believe those everyday steps deserve to be recognised because they are often the moments that create lasting change.

Tell us what you are looking for and we will help you understand the next step.
We empower people with support that strengthens independence, confidence and meaningful participation—because every person deserves the right support, at the right time, from the right people.
Our Story
AureonRise Care was founded with a simple purpose: to provide the kind of support we would want for our own families.
We recognised that participants and families were often looking for more than a service provider. They wanted consistency, genuine relationships, clear communication and support that reflected their goals.
Rather than fitting people into a system, we build support around each person’s unique journey. Relationships come first, support is personalised, and progress is defined by what matters to the individual.
Together, We Rise.

Common Questions
These answers explain how services generally begin and operate. Your actual arrangements will be confirmed in your service agreement and may depend on your plan, needs and the service requested.
Contact our team or submit a referral. We will ask about your goals, support needs, preferences, location, plan arrangements and preferred schedule. If the service is suitable and available, we explain the next steps, complete the required intake information and prepare a service agreement before support begins.
We can discuss self-managed, plan-managed and NDIA-managed arrangements. Whether a particular arrangement can be accepted depends on the service, AureonRise Care’s registration status for that support, current claiming requirements and the information available in the participant’s plan.
We consider the participant’s preferences, support requirements, communication style, worker skills, personality, cultural needs, location, schedule and availability. We aim for consistency, but matching is discussed openly and can be reviewed when the arrangement is not working well.
Your preferences are important. We discuss suitable options and make reasonable efforts to provide a compatible and consistent team. You can raise concerns or request a change, and we will review the request with you while considering safety, skills, availability and continuity of support.
We review the information you provide about your plan and the support requested. Where needed, we may clarify arrangements with you, your nominee, plan manager or support coordinator. The participant remains responsible for ensuring sufficient and appropriate funding is available unless another arrangement is documented.
Yes. The agreement should explain the support being delivered, responsibilities, schedule, rates, travel or other agreed charges, cancellation terms, privacy, feedback and how either party can change or end the service. You should have the opportunity to ask questions before signing.
Tell us as early as possible. We will review the requested change, discuss what is possible within the service scope and available funding, and update relevant support instructions or the service agreement when required. Changes involving clinical or high-risk support may require further assessment.
You can speak with a worker, contact the AureonRise Care office or use the contact details provided in your service information. Feedback and complaints should be handled respectfully, without affecting your right to receive safe support. You may also use an advocate or contact an external complaints body.
AureonRise Care’s routine services are not an emergency response service. Call Triple Zero (000) for an immediate threat to life or safety. For other urgent situations, follow the emergency and escalation contacts in your support plan and notify AureonRise Care as soon as it is safe to do so.

Good support begins with honest expectations. These questions help you understand how a provider works in practice—not only what appears in a brochure.
Write down the routines that are working, the areas that are difficult, the goals you want to move toward and any worker preferences that matter.
Helpful preparation: preferred days and times, communication needs, accessibility requirements, key contacts and the type of support relationship you want.A service agreement should clearly explain what will be delivered, the agreed price, responsibilities, cancellation arrangements, review periods and how either party can end the agreement.
Do not feel rushed. Ask for changes or clarification where wording does not reflect what was discussed. You can ask questions, involve a trusted person, request accessible communication, raise feedback, change your mind and seek support from an advocate.
Consent should be informed. You should understand what information is collected, why it is needed and who it may be shared with.NDIS Language Made Easier
These short explanations are general guidance and should be read alongside your current plan and official NDIS information.
A person who has an approved NDIS plan and can use the funded supports described in that plan.
Your right to make decisions about your goals, providers, workers and how your supports are delivered.
A registered plan manager pays providers and helps with invoices, while you retain flexibility in choosing supports.
You manage budgets, payments and records directly and have broader responsibility for purchasing supports.
The NDIA manages payments, and supports are generally purchased from providers eligible for agency-managed funding.
A written agreement setting out support arrangements, pricing, responsibilities, changes, cancellations and review processes.
Resource Questions
Yes, where the participant wants them involved. The participant’s preferences and consent should guide who receives information and takes part in decisions.
Participants have choice and control. Review the notice and cancellation terms in the current service agreement, then discuss a safe transition plan where continuity of support is important.
You may find it useful to bring a copy of relevant plan goals, preferred support times, communication information, important contacts and a short list of questions. Only share documents that are necessary.
Providers should explain what they collect, why it is required, how it is stored, who can access it and when consent is needed before sharing it.
Need help finding the right information?
Our team can explain our process, discuss your circumstances and point you toward the most relevant next step.
AureonRise Care works collaboratively with support coordinators to assess fit, clarify expectations and establish support arrangements that reflect the participant’s goals, preferences and circumstances.
We value timely communication, honest capacity discussions and practical information that helps everyone make confident decisions.
How We Work Together
The process is designed to confirm suitability before service commencement and reduce uncertainty for the participant and their support network.
We review the participant’s goals, requested supports, schedule, communication needs, preferences and relevant safety information.
We consider service location, timing, worker compatibility, required competencies and whether we can deliver the requested support responsibly.
Where there is a suitable match, we clarify responsibilities, pricing, service agreements, consent, key contacts and commencement planning.
We maintain agreed communication, respond to changes and review whether the support continues to align with participant goals.
A Faster Initial Review
A referral does not need to contain every record. The most useful information is current, relevant and connected to the support being requested.
Good-Fit Indicators
We welcome an early discussion where the participant or their network is looking for:
Our Communication Standard
Reliable partnerships depend on communication being clear before, during and after onboarding.
Speak with our teamWe will be transparent about availability, timing and limitations.
Requests for further information should be specific, relevant and explained.
Participants and coordinators should know who is doing what and by when.
Material changes, emerging concerns and delays should be raised early.
Discuss a Participant Referral
Send a referral or contact our team for an initial conversation before completing detailed paperwork.
Share the essential information needed for AureonRise Care to understand the request, review current capacity and determine whether a further conversation would be appropriate.
A referral does not guarantee service commencement. We will confirm fit, availability, consent and support requirements before any arrangement is agreed.
What Happens After Submission?
We confirm receipt and identify whether any essential information is missing.
We consider location, schedule, support type, participant preferences, worker requirements and relevant safeguards.
Where appropriate, we speak with the participant and authorised people to clarify expectations and consent.
This may involve onboarding, further assessment, a waitlist discussion or an honest explanation that we are not the right fit.
Whether you are exploring support for yourself, assisting a family member or making a professional enquiry, our team will listen, clarify what you need and explain the next step.
You do not need to have every detail ready. Start with what you know and the questions that matter most.
