Nursing Care
Nursing Care provides disability-related clinical support by appropriately qualified nursing professionals. Services are planned around assessed clinical needs, documented instructions, professional scope of practice and the participant’s preferences.
Who This Service May Suit
- Participants with disability-related clinical support needs included in their plan.
- People who require regular nursing assessment, monitoring or care planning.
- Participants transitioning between hospital, home and community-based supports.
- People whose support team needs clinical guidance or competency-based training.
Support May Include
- Clinical assessment, monitoring and development or review of nursing care plans.
- Medication-related support within professional scope and authorised instructions.
- Wound, skin-integrity, continence or catheter-related care where clinically appropriate.
- Enteral feeding, diabetes or other complex-care support when prescribed and within scope.
- Training and competency assessment for support workers completing delegated tasks.
- Communication with the participant’s health professionals and support network with consent.

How AureonRise Care Delivers It
A nurse reviews available clinical information, confirms consent and develops an agreed approach before delivering ongoing care. Clinical services are documented, reviewed and escalated when needed. We work within nursing scope of practice and do not replace the participant’s GP, specialist, hospital or emergency services.

Who delivers nursing support?
Clinical nursing services are delivered by appropriately qualified and registered nurses. The nurse’s experience and competencies must match the participant’s clinical requirements.
Can nursing support replace my doctor or hospital care?
No. Community nursing supports do not replace diagnosis, medical treatment, hospital care or emergency services. We coordinate with health professionals when consent and the service arrangement allow.
What should I do in a medical emergency?
Call Triple Zero (000) for an emergency. AureonRise Care’s routine nursing and disability support services are not an emergency response service.
