Support Coordination
Support Coordination helps you understand and implement your NDIS plan, connect with suitable providers and community supports, and build confidence managing your supports. The role is to strengthen your choice and control—not to make decisions on your behalf.
Who This Service May Suit
- Participants with Support Coordination funding in their plan.
- People starting a new plan or managing several providers and services.
- Participants experiencing service gaps, changes, complexity or barriers to implementation.
- People who want to build the skills to coordinate more of their supports independently.
Support May Include
- Explaining the plan, support budgets and how funded supports may be used.
- Helping identify and compare suitable providers and mainstream or community supports.
- Supporting service agreements, referrals and coordinated commencement of services.
- Monitoring plan implementation, service gaps, risks and progress towards goals.
- Preparing for plan discussions and gathering relevant provider information.
- Building the participant’s skills and confidence to manage supports over time.

Support Coordination Questions
What is the difference between a support coordinator and a plan manager?
A support coordinator helps you understand and organise supports and build your capacity. A plan manager focuses on paying provider invoices, financial administration and budget information. They are different roles.
Will you choose providers for me?
No. We can identify options, explain relevant information and support comparisons, but the participant retains choice and control over provider decisions.
Can I use providers outside AureonRise Care?
Yes. Support coordination should preserve your right to choose providers. Any real or perceived conflict of interest must be disclosed and managed transparently.
